We are seeking a compassionate *Speech-Language Pathologist (SLP)* to join our rehabilitation team in a Skilled Nursing Facility in Santa Rosa, CA.
Conduct comprehensive evaluations and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders. Provide evidence-based therapy, including dysphagia management and cognitive rehab, and perform optional swallow evaluations if credentialed. Complete accurate documentation in compliance with regulatory standards and track minutes for Medicare guidelines. Qualifications include a Master’s in Speech-Language Pathology, current California license, and preferred CCC-SLP certification. Experience in SNF or post-acute care is preferred.
