Speech-Language Pathologist (SLP) -Home Health- Sonoma County

The Speech–Language Pathologist is responsible for delivering speech therapy services, including evaluations, developing care plans, administering therapy, recommending communication aids, and collaborating with healthcare teams to ensure comprehensive patient care.

  • Conducts assessments, identifies speech defects, and plans interventions.
  • Administers therapy using specialized techniques and equipment.
  • Recommends communication devices and educates patients, families, and staff.
  • Coordinates with physicians and participates in meetings for integrated care.
  • Maintains documentation, stays current in clinical practices, and refers patients to additional services as needed.
  • Supervises aides when appropriate and performs other related duties.
  • Qualifications include a master’s or doctoral degree in speech-language pathology, licensure or equivalent credentials, clinical experience, and good communication skills. The role offers part-time or per diem work at $30+ per hour, with benefits such as insurance, flexible scheduling, and paid time off.
