Role Overview: The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation to patients in a skilled nursing facility setting. They assess, develop, and modify treatment plans to help patients achieve functional goals and improve quality of life. The role involves collaborating with interdisciplinary teams, documenting progress, and ensuring compliance with laws and standards.
Responsibilities include:
- Assessing and treating patients with speech, language, and cognitive disorders
- Overseeing and evaluating care, including supervising CFYs (if applicable)
- Planning and modifying therapy goals, documenting progress, and utilizing therapy software
- Participating in community health initiatives and maintaining professional relationships
- Adhering to safety, confidentiality, and company policies
Qualifications & Skills:
- Graduate of an accredited speech therapy program (MS or MA)
- ASHA certification (CCC) or eligible for Clinical Fellowship Year (CFY)
- Active state licensure and CPR certification
- Proficiency in Microsoft Office and communication skills
- Knowledge of laws, regulations, and best practices in post-acute care
- Ability to make independent decisions, demonstrate professionalism, and work effectively in a team
