We are seeking a licensed Speech Language Pathologist (SLP) for a Part-time/PRN role to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.
The role involves working with patients recovering from stroke, TBI, illness, surgery, injury, or dementia, focusing on positive outcomes and personalized care.
Responsibilities include:
- Conducting initial assessments
- Developing and coordinating individualized treatment plans
- Collaborating with the care team for seamless patient care
- Documenting notes in the health information system
Requirements:
- Current SLP license
- Valid driver’s license
- Ability to relocate to State College, PA before starting
