Part-time/PRN opportunity for a licensed Speech Language Pathologist (SLP) to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.
Responsibilities include conducting initial assessments, developing individualized treatment plans, collaborating with the care team to ensure continuity of care, and recording notes in the health information system.
Must have current SLP licensure and a valid driver’s license. The role involves working with patients who may have cognitive-communication or swallowing issues due to stroke, brain injury, illness, surgery, injury, or dementia.
