Part-time/PRN licensed Speech Language Pathologist (SLP) position assessing, diagnosing, treating, and preventing communication and swallowing disorders in patients’ homes.
Responsibilities include:
- Initial patient assessments
- Developing individualized treatment plans
- Collaborating with the care team to ensure continuous care
- Documenting notes in the health information system
