Role: Home health Speech Language Pathologist (SLP) performing evaluations and therapeutic interventions for adult and geriatric patients in clients’ homes.
Responsibilities:
- Conduct home visits within designated areas.
- Perform assessments to diagnose speech, language, communication, and swallowing issues.
- Develop, implement, and revise personalized treatment plans based on evaluations, physician’s orders, and client goals.
- Provide rehabilitative care and patient, family, or caregiver education to promote health, safety, and independence.
- Accurately document observations, interventions, and evaluations using electronic medical records.
Qualifications & Skills:
- Current Vermont Speech Language Pathologist license.
- Certification from the American Speech and Hearing Association or equivalent educational and work experience.
- Experience with electronic medical records preferred.
- Strong communication, assessment, and documentation skills.
