Provide personalized, one-on-one speech therapy services in patients’ homes, focusing on functional improvement.
Conduct evaluations, assess home environments, and identify equipment needs.
Develop and implement care plans, administer therapy, and recommend communication aids.
Collaborate with physicians and healthcare team members to ensure coordinated care.
Educate patients, families, and caregivers, and refer to additional services as needed.
Maintain clinical competency and participate in meetings to ensure quality care.
Supervise home health aides when appropriate and complete documentation per policy.
Qualifications: Master’s or doctoral degree in speech-language pathology, state licensure or supervised clinical hours, national exam completion, and at least one year of clinical experience.
