Role: Speech Language Pathologist providing in-home evaluation and treatment for adults and seniors in community-based care programs.
Responsibilities:
- Assess speech, language, cognitive-communication, and swallowing disorders.
- Develop person-centered care plans tailored to individual needs.
- Deliver evidence-based therapy to promote safety and independence at home.
- Collaborate with nurses, therapists, caregivers, and families to ensure comprehensive care.
- Educate clients and caregivers on strategies and equipment use.
- Document progress in compliance with regulatory and payer standards.
