Social Worker (LMSW or LLMSW)

The Master Social Worker will coordinate social services for elderly participants in a comprehensive care program, conducting in-home assessments, developing care plans, managing community resources, and facilitating family meetings.

Must hold an MSW license in Michigan with at least one year of experience working with the elderly or long-term care populations.

Requires strong communication, organizational skills, and proficiency in MS Office.

Responsibilities include Advance Care Planning, caregiver education, and ensuring confidentiality.

Must be vaccinated against specified diseases and meet CMS standards.
