Role Summary
Provide residents with social, emotional, and psychological support in a skilled nursing facility.
Key Responsibilities
- Support residents and families through therapeutic, compassionate engagement.
- Help meet residents’ social and emotional needs to promote well-being.
- Maintain accurate, required documentation and records.
- Participate in care planning to support resident-centered outcomes.
- Coordinate discharge planning to assist residents with smooth transitions from the facility.
