Seeking a skilled Social Services Clinician for discharge planning and care coordination at a Level IV Critical Access Hospital in Southeast Alaska.
Responsibilities include conducting assessments, brief therapy, and treatment planning for patients from birth to elder, facilitating care conferences, and supporting mental health and chemical dependency needs.
Coordinate with hospital teams and community resources to ensure smooth care transitions, advocate for clients, and assist with complex matters like Power of Attorney.
Maintain accurate documentation in compliance with regulations, participate in team meetings, and contribute to quality improvement efforts.
Qualifications include a Master’s in Social Work, Psychology, or related field, licensure or in-process licensure as LCSW, LPC, or LMFT, and clinical competency within 3 months of hire. Valid driver’s license required.
