Social Services Clinician – In-Patient – Discharge Planning

Seeking a Social Services Clinician for a Level IV Critical Access Hospital in Southeast Alaska.

Responsibilities include conducting clinical assessments, brief therapy, and discharge planning for diverse patient populations, from birth to elders.

Engage in care coordination, advocacy, and collaboration with hospital teams and community partners to ensure seamless care transitions.

Maintain accurate documentation in compliance with regulatory standards and participate in team meetings, trainings, and quality initiatives.

Qualifications include a Master’s degree in Social Work, Psychology, or related field, with licensure or in-process licensing as LCSW, LPC, or LMFT. Must have clinical competency within 3 months and knowledge of discharge planning, crisis intervention, and legal/ethical practices.

Offers a pay range of $36.46-$51.29/hour ($82K-$106K/year), with a $10K sign-on and $10K relocation bonus. Benefits include retirement, health insurance, paid time off, parental leave, and more.
