Social Services Clinician – In-Patient – Discharge Planning

Seeking a Social Services Clinician for a Level IV Critical Access Hospital in Southeast Alaska.

Responsibilities include conducting assessments, diagnosis, brief therapy, and discharge planning for patients from birth to elder, facilitating care conferences, and providing care coordination and advocacy.

Key duties involve maintaining compliant documentation, collaborating with hospital teams and community partners, and ensuring smooth care transitions.

Must hold a Master’s degree in Social Work, Psychology, Counseling, or related field, with licensure or in process of licensure (LCSW, LPC, or LMFT).

Requires knowledge of discharge planning, crisis intervention, and legal/ethical practices, along with skills in case management, documentation, and assessment.
