Seeking a licensed Social Services Clinician for discharge planning at a Level IV Critical Access Hospital in Southeast Alaska.
Provide assessments, diagnoses, brief therapy, and care planning for patients from birth to elder. Lead discharge planning, facilitate care conferences, and support mental health and chemical dependency needs. Coordinate with hospital teams and community partners to ensure appropriate services and smooth care transitions. Maintain accurate documentation compliant with regulations and standards. Advocate for clients, assist with skill-building, and manage complex matters like Power of Attorney. Participate in team meetings, staff training, and quality improvement activities. Requirements include a Master’s degree in a related field, active licensure (LCSW, LPC, or LMFT), and clinical competency within 3 months of hire.
