Skilled Nursing Facility Rounder provides comprehensive medical care for residents in post-acute and long-term care settings.
- Conduct daily clinical rounds and perform resident assessments.
- Manage acute and chronic conditions, reviewing diagnostic results and adjusting medications as needed.
- Coordinate and update patient care plans with nursing, therapy, and social work teams.
