Senior Social Worker – Dialysis

Join a dedicated interdisciplinary team as a Dialysis Social Worker, providing compassionate, patient-centered support to improve the lives of dialysis patients.


Key responsibilities include:

  • Applying advanced social work skills to assess psychosocial needs and respond to abuse, neglect, or violence following ethical guidelines and laws.
  • Conducting in-depth psychosocial assessments and providing specialized counseling and support.
  • Collaborating with healthcare team members to develop and implement effective treatment and care plans.
  • Supporting patients and families through emotional, social, and economic challenges, facilitating optimal functioning.
  • Utilizing age-specific care protocols for diverse patient populations, from neonatal to geriatric.
  • Possessing knowledge of psychosocial and economic issues related to illness, with the ability to work independently and as part of a team.

Qualifications include:

  • A master’s degree in Social Work from an accredited program.
  • Current licensure in good standing (e.g., LGSW or LICSW in Minnesota).
  • Preferred experience in healthcare or mental health settings.

Skills required: critical thinking, psychosocial assessment, counseling, teamwork, and understanding of illness impacts on patients and families.
