RN Care Specialist will coordinate and deliver high-quality nursing care for seniors across skilled nursing, rehab, assisted living, and memory care.
Key Duties
- Perform comprehensive nursing assessments and reassessments.
- Develop, implement, and update individualized care plans.
- Coordinate care with an interdisciplinary team to support person-centered outcomes.
- Educate residents and families on care plans and health needs.
