We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting.
Serve as a clinical leader, collaborating with the care team to manage short- and long-term residents. Perform admissions, discharges, monthly assessments, and evaluate residents for condition changes. Handle advanced care planning, comprehensive assessments, and prescribe treatments.
