Senior Nurse Practitioner – Skilled Nursing Facility – Altira at Inners Creek

We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting.

Serve as a clinical leader, collaborating with the care team to oversee residents’ short- and long-term needs.

Perform admissions, discharges, stability checks, and evaluations for changes in condition.

Handle advanced care planning, comprehensive assessments, and prescribe treatments.

Requirements:

Active Pennsylvania APRN license and DEA.
