Full-time home health Physical Therapist role with a $15,000 signing bonus.
Role Overview:
Provide patient-centered physical therapy services in a home health setting, facilitating recovery from illness or injury. Conduct initial and ongoing assessments to evaluate mobility, strength, and function, and develop individualized treatment plans with measurable goals. Deliver therapy in accordance with physician orders, document clinical progress, and modify plans as needed. Collaborate with healthcare teams, educate patients and caregivers on home exercise programs, and participate in discharge planning. Supervise other personnel (PTA, HHA, OTA) where allowed, and complete OASIS assessments per regulations. Maintain compliance with policies, standards, and legal requirements, ensuring effective communication with staff, patients, and families.
Qualifications & Skills:
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license as a Physical Therapist.
- Valid CPR, driver’s license, and proof of auto insurance.
Preferred Experience:
- Home health therapy experience and OASIS documentation skills.
