Position Overview
Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within home health care. Responsibilities include conducting assessments of mobility, strength, and function; developing and updating plans of care; documenting client progress; and collaborating with healthcare teams, patients, and caregivers. The role involves supervising personnel like OT Assistants and Home Health Aides, completing assessments such as OASIS where permitted, and ensuring compliance with documentation and regulatory standards.
Qualifications:
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license
- Valid CPR certification

- Attention to detail and relationship-building abilities
- Experience in home health therapy and OASIS documentation
Key Duties:
- Perform initial and ongoing assessments
- Provide therapy services per physician orders
- Maintain accurate documentation and modify care plans as needed
- Coordinate services with healthcare team and educate patients and caregivers
- Supervise relevant personnel and complete assessments within regulatory scope
