What You’ll Do:
Perform assessments for admissions/readmissions and monitor changing resident conditions.
Educate residents and families, communicate with providers, and manage clinical procedures like line management and urinary catheter changes.
Support interdisciplinary treatments, report events, and develop performance improvement plans.
Maintain accurate documentation, care plans, and respond to emergencies.
Mentor staff and ensure optimal care delivery aligned with resident preferences.
Qualifications:
Active PA RN license and BSN from an accredited school.
Current CPR/BLS; wound care and infusion therapy certifications preferred.
Minimum 3 years long-term care and 2 years acute care management experience.
