Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventative care, conduct screenings, coordinate disease management, and support primary clinical activities. The nurse will assess patient needs, develop individualized care plans, facilitate communication among healthcare providers, and educate patients and families on disease and case management.
Key Responsibilities:
- Assess healthcare, educational, and psychosocial needs of patients and families.
- Coordinate diagnostic tests and treatments, ensuring adherence to clinical guidelines.
- Serve as a resource for disease management and patient education.
- Implement clinical interventions, monitor therapies, and follow up as directed.
- Collaborate with providers to improve evidence-based practice and promote best outcomes.
- Maintain documentation in electronic health records and collect relevant data.
- Participate in professional development and quality improvement initiatives.
- Assist with specimen collection, medication administration, and IV management within scope of practice.
Qualifications & Skills: Requires a graduate of a Specialty Training Program in Nursing, with at least 2 years of RN experience; Med/Surg hospital experience preferred. Strong communication, clinical assessment, and collaboration skills are essential.
