Role: RN Patient Care Navigator – Stroke Program
Responsibilities: Provide professional nursing care to support neurologists and physicians in managing stroke patients across the stroke continuum. Focus on acute stroke alerts, emergency interventions, and collaborating with multidisciplinary teams to ensure timely, evidence-based care. Perform neurological assessments, follow treatment protocols, assist with patient transport, and serve as a resource for stroke protocols, quality improvement, and regulatory compliance. Educate patients, families, and staff on stroke care and maintain stroke program data to meet center standards.
Qualifications:
- Bachelor’s Degree in Nursing
- Minimum three years RN acute care experience in stroke treatment
- Current Illinois RN licensure
- CPR (BLS), ACLS (or obtain within 3 months), and NIHSS certifications
Preferred Skills: Critical care or inpatient stroke care experience, strong communication skills, ability to remain calm under stress, and delivering care to acute stroke patients.
