Role Overview:
The Nurse Navigator is a leadership position supporting direct patient care providers by coordinating and ensuring high-quality, safe, and effective care. They assess patient needs, facilitate care plans, and serve as a resource for the care team.
Key Responsibilities:
- Manage patient care from pre-op to recovery and follow-up.
- Coordinate discharge planning and post-acute services.
- Provide patient and family education.
- Conduct inpatient rounding and monitor patient outcomes.
- Collaborate with healthcare team members, maintain documentation, and support program growth.
Qualifications & Skills:
- Graduate of an accredited nursing program with an RN license.
- BLS certification; 2–3 years nursing experience.
- Preferred: BSN, ACLS, orthopedic or care coordination experience.
- Physical ability to lift up to 50 lbs, stand, walk, and perform bending or reaching activities.
Additional Details:
The role involves mentoring staff, supporting quality metrics, and collaborating with physicians and support services to optimize patient outcomes within a specialty orthopedic surgery setting. The position offers a day shift with no nights, weekends, or holidays.
