Role: Provides advanced nursing support within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management, supporting primary clinical functions for patients, families, and providers.
Responsibilities:
- Assess healthcare, educational, and psychosocial needs; develop individualized care plans.
- Coordinate diagnostic tests and facilitate communication among patients and healthcare team.
- Educate patients and families on disease management and care coordination using age-appropriate strategies.
- Implement clinical interventions based on risk and evidence-based guidelines, collaborating with providers to adjust therapies and ensure adherence.
- Maintain documentation, manage patient interactions, and assist with triage and clinical workflows.
- Administer medications, vaccines, and IVs within scope of practice, and prepare laboratory specimens.
- Participate in professional development and post-discharge care coordination to improve patient outcomes.
Qualifications & Skills:
- Minimum of 2 years RN experience.
- Graduate from Specialty Training Program-Nursing (required), Bachelor’s preferred.
- Strong clinical, communication, and organizational skills.
- Ability to work collaboratively within a team in a clinical setting.
