Job Summary: This role involves coordinating and delivering infusion therapy services across clinical settings, including patient education, case management, and collaboration with healthcare teams, primarily covering multiple hospital sites.
Responsibilities: Manage patient referrals, develop individualized care plans, prepare and administer infusion medications, and perform vascular access procedures. Educate patients and caregivers, monitor therapy progress, ensure documentation accuracy, and support quality improvement initiatives. Participate in on-call rotations and provide clinical oversight to LPNs. Travel to patient homes and clinical sites to deliver care and assessments.
Qualifications & Skills: Minimum of 3 years RN experience, with preferred expertise in home health and IV therapy. Graduate from a Specialty Training Program in Nursing. Ability to work independently, communicate effectively, and follow clinical protocols.
Key Duties: Ensure safe, effective, patient-centered infusion care, maintain compliance with standards, respond to patient concerns, and contribute to protocol development. Uphold safety and quality standards in a clinical environment.
