Role: Responsible for accepting Hospice, Home Health, and Home Infusion referrals in various healthcare settings, including acute care, physician clinics, nursing homes, and rehabilitation centers.
Key Duties: Maintains an active presence in practice areas, provides excellent customer service to discharge planners, social workers, physicians, and patients, and facilitates timely discharges to ensure continuity of care within the health system.
Qualifications: Registered Nurse license required with a preference for bachelor’s degree. Minimum of three years nursing experience, ideally in Hospice, Home Health, or acute settings. Knowledge of regulatory guidelines, strong assessment, communication, and documentation skills. Self-directed, team-oriented, flexible, and capable of working with minimal supervision.
Preferred Certifications & Experience: Certification in Hospice and Palliative Nursing (CHPN) and experience in hospice or home nursing are a plus.
