RN – Hospice

Overview

This role involves providing direct and indirect quality care to patients and families of all ages within a hospice setting, following the nursing process and Nurse Practice Act. The nurse manages a caseload, coordinates care with the healthcare team, and ensures patient comfort and dignity.

Key Responsibilities:

  • Deliver patient-centered hospice care in home and facility settings.
  • Collaborate with healthcare team to promote comfort and peace at end of life.
  • Travel to patient locations within designated counties.
  • Manage a patient caseload, ensuring individualized care plans.

Qualifications & Skills:

  • Oregon Registered Nurse License and BLS certification.
  • Minimum of 1 year medical/surgical, acute care, or long-term care experience.
  • Experience in end-of-life (EOL) care; recent oncology, hospice, or home health experience preferred.
