Role Overview:
The RN Home Health provides comprehensive patient assessments, develops individualized care plans, and coordinates services for homebound patients. Responsibilities include performing routine and complex nursing procedures, educating patients and caregivers to promote self-care, and communicating with the healthcare team to ensure appropriate care delivery. The role involves adherence to regulatory guidelines, accurate documentation, participation in performance improvement activities, and supporting staff development. The RN collaborates closely with physicians, family members, and team members to meet patients’ physical, emotional, and educational needs.
Qualifications & Skills:
– Licensed as a Registered Nurse in Georgia
– Minimum 1 year of Medical/Surgical, Home Health, or ICU experience
– Strong critical care nursing skills
– Certification in BLS and valid Georgia driver’s license with insurance
– Computer proficiency with data entry, Word, Excel; Epic and 3M experience preferred
– Excellent communication and teamwork abilities
