Role Overview:
This position is for a PRN Home Health Registered Nurse responsible for delivering skilled nursing services in patients’ homes within a specified region. The role involves evaluating patient needs, developing and implementing individualized care plans, and coordinating care with an interprofessional team. The nurse promotes respectful, culturally sensitive patient interactions, monitors patient condition changes, and updates care plans accordingly. The position requires working a minimum of two full shifts in a six-week period, with typical hours Monday through Friday, 8 am to 4:30 pm, including holiday rotation.
Responsibilities:
- Assess patient preferences, needs, and barriers to care, considering emotional, spiritual, and cultural factors.
- Develop, document, and communicate personalized care plans with measurable goals, ensuring seamless care coordination.
- Deliver compassionate, equitable care supporting patient self-care and cultural diversity.
- Evaluate patient condition changes, collaborate with families and team members, and update care plans in electronic records.
- Practice safety-sensitive procedures, maintaining alertness and safety at all times.
Qualifications & Skills:
- Minimum education: Nursing diploma or associate’s degree in Nursing.
- Experience: At least 2 years of nursing experience; preferred Bachelor’s degree in Nursing or Home Health with 2-5 years of relevant experience.
- Licenses & Certifications: Valid RN license, driver’s license.
- Skills: Critical thinking, cultural competence, excellent communication, and teamwork abilities.
