The RN Case Manager provides primary care to 25-35 home-based patients, addressing physical, emotional, developmental, psychosocial, and educational needs. Key responsibilities include assessing patient needs, developing and adjusting care plans, supervising LPNs and Home Health Aides, and coordinating multidisciplinary services to ensure positive outcomes. They communicate with physicians, patients, families, and team members, ensuring accurate documentation, adherence to regulatory guidelines, and participation in performance improvement activities. Qualifications include a nursing degree from an accredited program, RN licensure (multi-state preferred), BLS certification, and at least one year of Medical/Surgical nursing experience, with preferred home health and ICU background. Skills required are strong clinical, leadership, organizational, and communication abilities, with proficiency in home health operations, regulatory compliance, and documentation systems. Additional duties involve patient education, staff training, and contributing to quality improvement initiatives.
