Role: The RN Care Manager II – IPCM collaborates with an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring safe transitions post-hospitalization.
Responsibilities: Develops individualized care plans, communicates effectively with patients, families, and healthcare providers, and advocates for patient needs. Promotes health education and facilitates smooth care transitions across settings.
Qualifications & Skills: Requires a graduate of an accredited Nursing program, with a Bachelor’s in Nursing. Must hold a current RN license, BLS certification, and have at least 1 year of acute care hospital experience. Preferred certifications include Case Management, with additional experience in care or utilization management. Clinical expertise, critical thinking, and compassion are essential.
