RN Acute Care Transition Specialist

Role: Provides direct patient care in a high-acuity acute care setting, utilizing the nursing process within a multidisciplinary team.


Responsibilities: Prioritizes patient wellness, safety, and evidence-based practices; models proficiency through precepting; contributes to quality care; and supports a fast-paced, innovative clinical environment.


Qualifications: Graduation from an accredited nursing program, RN licensure, BLS certification, and at least 6 months of nursing experience. A Bachelor’s degree in Nursing is preferred.


Skills & Key Duties: Demonstrates top-of-scope practice, employs clinical standards, and participates in specialty units such as Med-Surg. Emphasizes safety, patient outcomes, and professional growth through ongoing learning and certification.
