Role: LPN Rehab Nurse
Provide compassionate, hands-on nursing care for seniors in a rehabilitative setting. Administer medications and treatments, perform focused assessments, and support residents in achieving therapy and recovery goals.
Key Responsibilities
- Deliver and document medications and prescribed treatments.
- Conduct focused resident assessments and monitor safety and comfort.
- Collaborate with RNs, therapists, physicians, and families to implement individualized care plans.
- Document resident progress and communicate changes to the care team.
- Support recovery goals through patient-centered, dignified care.
Qualifications & Skills
- Licensed Practical Nurse (LPN) with strong clinical competence in rehab/senior care.
- Knowledge of medication administration, treatment delivery, and nursing documentation.
- Strong communication and teamwork skills for multidisciplinary collaboration.
- Compassionate approach with commitment to safety, comfort, and resident dignity.
