Summary:
This role involves providing direct patient care within a community-based, multi-specialty organization focused on value-based healthcare. Responsibilities include assessing patients, performing triage, managing basic life support, conducting diagnostics, and documenting patient information in the electronic medical record. The nurse collaborates with healthcare team members to develop and evaluate care plans, supports patients through pre- and post-visit calls, and occasionally visits clinical sites to coordinate care.
