Registered Nurse (RN) / Licensed Practical Nurse (LPN) | Long Term Care

Role Overview:


Provides comprehensive nursing care to residents, including developing care plans, administering medications, and monitoring health conditions. Collaborates with the healthcare team, communicates effectively with residents, families, and physicians, and ensures adherence to safety and infection-control protocols. Educates residents and families on health management and self-care, evaluates care outcomes, and adjusts plans as needed. Coordinates admissions, discharges, and transfers while supervising nursing staff to maintain an efficient workflow. Maintains a safe, secure, and compassionate environment aligned with organizational standards.


Qualifications & Skills:

  • Valid RN license in the state of practice.
  • Ability to perform essential functions with adequate vision and communication skills.
  • Preferred 1-3 years clinical or long-term care experience.
  • Commitment to organizational values, confidentiality, and teamwork.
  • Knowledge of safety, legal regulations, and infection control standards.

Key Responsibilities:

  • Review physician prescriptions and develop individualized care plans.
  • Coordinate with healthcare providers and communicate care details.
  • Teach residents and families about health conditions and self-care.
  • Monitor and evaluate resident progress, modifying care plans accordingly.
  • Maintain safety, cleanliness, and proper functioning of care equipment and supplies.
  • Assist with admissions, discharges, and transfers.
  • Supervise and coordinate nursing staff workflow.
