Role: RN Case Manager providing compassionate end-of-life care in patients’ homes within Bloomington, Pontiac, and nearby areas.
Responsibilities:
- Conduct comprehensive, ongoing patient assessments to determine evolving needs.
- Develop and adjust personalized care plans in collaboration with physicians and care teams.
- Administer medications and treatments per physician orders.
- Maintain accurate and timely clinical documentation.
- Serve as a communication link among families, physicians, and community health partners.
- Educate patients and caregivers to foster confidence and understanding.
- Participate in on-call rotations as per team policies.
Qualifications & Skills:
- Active RN license in the state of practice.
- Graduate of an accredited Nursing program.
- Preferably 1+ years of hospice, home health, or related experience.
- Strong clinical judgment and ability to work independently.
- Excellent communication skills with care teams and families.
- Compassionate approach to end-of-life care.
