Registered Nurse (RN) – Acute Care – Care Coordination Supervisor – Full-Time Days

Role: Care Coordination Daytime Supervisor (RN) is responsible for overseeing hospital care coordination during acute hospitalization, ensuring seamless patient transitions and optimal outcomes.

Key Responsibilities:

  • Lead and mentor care team members in assessment, discharge planning, and patient advocacy.
  • Coordinate with multidisciplinary teams and community agencies to facilitate timely transfers and follow-up care.
  • Support quality improvement initiatives, monitor performance, and ensure documentation accuracy.
  • Manage staffing, provide coaching, and foster a culture of trust and engagement.
  • Address complex issues, conduct assessments, and communicate effectively with patients, families, and providers.
  • Qualifications & Skills:

  • Minimum of 3 years recent hospital care coordination experience, with at least 2 years in an acute care setting.
  • Bachelor’s degree in Nursing (BSN) or Master’s in Social Work (MSW); active licensure (e.g., RN, LMSW, LCSW).
  • Certifications such as BLS, CCM or ACM preferred.
  • Strong interpersonal, communication, and critical analysis skills.
  • Knowledge of case management processes, community resources, and payor regulations.
  • Ability to work independently, multitask, and promote client autonomy in a fast-paced environment.
  • Additional Duties:
    – Collaborate with hospital leadership and physicians to improve service delivery.
