Role Overview: The RN Nurse Navigator Triage provides clinical triage services via phone and in person, assessing patient acuity, delivering health advice, and coordinating care.
Key Responsibilities: Perform patient assessments, prioritize urgent calls, provide education, schedule emergency appointments, and coordinate with departments and external organizations. Ensure timely documentation, communicate effectively with patients and providers, and assist with training new staff. Manage patient concerns, explain medical conditions, and support service improvement efforts.
Qualifications & Skills: BSN preferred, with at least 5 years of nursing experience, especially in ICU, ED, or Critical Care. Proficiency in computer skills and Microsoft Office is required. Certification in specialty areas is a plus.
Work Environment & Schedule: Typically performed in a clinical setting, with rotating shifts including days and nights. May involve minimal travel for training or meetings.
