Registered Nurse Navigator – Nephrology

Role Overview: Provides care coordination, patient education, and ongoing support for patients entering multidisciplinary health programs. Acts as a liaison between patients and healthcare teams, facilitating care transitions, and ensuring continuity of care.

Key Responsibilities:

  • Facilitates patient entry into care programs and coordinates pre- and post-hospital care.
  • Serves as a patient navigator, guiding new patients and promoting understanding of their conditions.
  • Collaborates with healthcare providers to develop and implement individualized care plans.
  • Provides clinical support, educates patients and families, and coordinates activities with support services like support groups and social agencies.
  • Maintains follow-up systems, ensures timely surveillance, and supports physicians during procedures.
  • Acts as a resource and educates team members in specialized areas.

Qualifications & Skills: Minimum of 2 years relevant experience; graduation from a specialty training program (required) with a preference for a Bachelor’s in Nursing. Strong communication, coordination, and patient education skills are essential.
