Position Overview:
Join as a Home Infusion Nurse to deliver expert infusion therapy in patients’ homes and community settings, fostering meaningful patient relationships and promoting safe, patient-centered care.
Key Responsibilities:
- Coordinate intake, verify referrals, and develop individualized care plans based on comprehensive assessments.
- Administer infusion medications using aseptic technique, perform vascular access procedures, and conduct lab draws.
- Educate patients and caregivers on therapy protocols and equipment use.
- Monitor therapy progress, adjust care plans, and communicate with healthcare providers for optimal outcomes.
- Ensure accurate documentation, compliance with protocols, and participate in quality improvement initiatives.
- Provide clinical oversight to LPNs and support 24/7 care coverage through on-call duties.
- Travel to patient homes and clinical sites, delivering care and patient education.
Qualifications & Skills:
- Minimum of 3 years RN experience; home infusion and home care experience preferred.
- Graduate from Specialty Training Program-Nursing (Required).
- Strong clinical assessment, communication, and organizational skills.
