Role Overview: This position involves managing patient care by assessing, planning, implementing, and evaluating individualized treatment plans to optimize health outcomes. The nurse collaborates with patients, families, and healthcare teams to coordinate services, monitor progress, and ensure quality and efficiency in care delivery.
Key Responsibilities:
- Develops patient-centric care plans integrating evidence-based guidelines and protocols.
- Monitors laboratory results, diagnostics, and utilization patterns to evaluate care quality.
- Promotes early intervention through systems of care and population health strategies.
- Works collaboratively with the healthcare team to ensure seamless service coordination.

Qualifications & Skills:
- Minimum of two years RN experience, preferably in a hospital setting.
- Case management experience is a plus.
- Graduate from a nursing specialty training program; Bachelor’s degree preferred.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work on-site and adapt to a team-oriented environment.
