Registered Nurse (Home & Community-Based Care)
Provide compassionate, skilled nursing care in clients’ homes, supporting safety, independence, and dignity for seniors and individuals with disabilities.
Key Responsibilities
- Conduct initial and ongoing nursing assessments and monitor client health status.
- Create, implement, and update individualized care plans based on client needs.
- Administer medications and perform prescribed treatments and nursing interventions.
- Coordinate care with physicians, therapists, and family/caregiver support systems.
- Promote a client-centered approach while ensuring safe delivery of care.
Qualifications & Skills
- Current RN licensure and strong clinical nursing judgment.
- Experience with home/community health nursing and individualized care planning.
- Ability to assess, document, and adapt care plans as client needs change.
- Excellent communication, teamwork, and collaboration skills.
- Compassionate, patient-focused demeanor with attention to safety and compliance.
