Registered Nurse – Home Health

Role Overview:

A home health registered nurse provides one-on-one patient care, supporting an underserved community while maintaining work-life balance and flexible scheduling. This role involves building meaningful patient relationships, assessing needs, developing and implementing personalized care plans, and collaborating with an interprofessional team to ensure quality, evidence-based care. The nurse evaluates patient conditions, communicates updates, and advocates for patient-centered continuity of care.

Qualifications include a Nursing Diploma or Associate’s degree, RN licensure, and a valid driver’s license. Preferred candidates have a Bachelor’s in Nursing and 2-5 years of experience in home health. The position requires the ability to provide care across all regions during on-call shifts and to work safely and alertly at all times.
