Role Overview
This position involves providing specialized nursing services through home visits, evaluating patient needs, developing and implementing personalized care plans, and collaborating with an interprofessional team. Qualifications & Skills
- Minimum of an Associate’s degree in Nursing (RN license required)
- At least 2 years of nursing experience; 2-5 years preferred for advanced roles
- Valid Driver’s License and registration with the Family Care Safety Registry
- Strong assessment, communication, and critical thinking skills
- Ability to work collaboratively and adapt to diverse patient needs
