Role: PRN Registered Nurse Case Manager
Coordinate patient care across the continuum in a patient-centered hospital setting. Assess clinical and psychosocial needs, develop individualized care plans, and manage utilization to support appropriate care delivery.
Key Duties:
- Conduct comprehensive assessments and formulate individualized care plans
- Coordinate interdisciplinary care and support safe, timely discharges
- Manage utilization and facilitate appropriate levels of care
- Educate patients and families on care plans and next steps
- Connect patients with community resources and supportive services
Qualifications & Skills:
- Current Registered Nurse licensure
- Strong clinical judgment and care coordination skills
- Ability to assess both clinical and psychosocial needs
- Effective communication and collaboration with interdisciplinary teams
- Patient education and resource coordination capabilities
