RN Care Specialist
Coordinate and deliver high-quality, person-centered nursing care for seniors across skilled nursing, rehab, assisted living, and memory care. Conduct nursing assessments, develop and update care plans, and collaborate with an interdisciplinary team to support residents’ clinical needs.
Key Duties
- Perform comprehensive nursing assessments and monitor resident health status
- Develop, implement, and regularly update individualized care plans
- Provide direct nursing care and coordinate ongoing clinical needs
- Educate residents and families and support informed care decisions
- Advocate for dignity, compassion, and quality outcomes
- Ensure compliance with applicable regulatory and care standards
Qualifications & Skills
- Current RN licensure
- Strong clinical assessment and care planning skills
- Excellent communication and teamwork within an interdisciplinary setting
- Commitment to person-centered care and patient advocacy
- Knowledge of regulatory compliance requirements in senior care settings
