Role: Home Health Registered Nurse
Responsibilities:
Perform comprehensive assessments of adult and geriatric patients in their homes
Manage wound care, IV therapy, and medication administration
Develop and implement individualized care plans to promote patient safety and independence
Collaborate with an interdisciplinary team to coordinate care
Educate patients and families on health management and safety protocols
Ensure documentation and care comply with regulatory and quality standards
Qualifications & Skills:
Licensed Registered Nurse with relevant certifications
Strong clinical assessment and wound care skills
Ability to develop personalized care plans
Excellent communication and patient education skills
Team-oriented with a commitment to quality and compliance
